The Icelandis Transportation Investigation Board has issued a report regarding an aviation accident that occurred at Keflavik Airport on July 21st 2013, when Sukhoi Civil Aircraft RRJ-95B incurred a runway excursion. The airplane was undergoing flight testing on behalf of its manufacturer and the flight crew intended to execute a go-around at 2-3 feet radio altitude over RWY 11, under crosswind conditions, near the airplane‘s maximum landing weight, with one engine inoperative. The purpose of the flight was to test the airplane‘s automatic flight control system under these conditions.
The Icelandis Transportation Investigation Board determined that the most probably cause of the accident to be flight crew fatigue, which resulted in the flight crew advancing the incorrect throttle lever to TO/GA position. This occured after the airplane touched the runway, which had resulted in the automatic flight control system shutting off.
The Icelandis Transportation Investigation Board issues nive safety recommendation in the report. Three for the manufacturer (Sukhoi Civil Aircraft), three to the operator of Keflavik Airport (Isavia), one to the Icelandic Transportation Authority, one to the Russian Ministry of Industry and Trade and one jointly to EASA and the Interstate Aviation Committee. In addition one safety action is issued to the Icelandic Ministry of Interior.
